Rhode Island Department of Health
The Chronic Disease Self-Management Education Program housed in the Chronic Care and Disease Management Team at RIDOH supports health-related evidence-based programs funded by the Centers for Disease Prevention and Control’s Arthritis grant and the Administration for Community Livings’ Chronic Disease Self-Management Education grant.
Rhode Island Parent Information Network
The Rhode Island Parent Information Network (RIPIN)is a 501 (c)(3), charitable, nonprofit organization established in 1991 by a passionate group of parents of children with special needs. These parents recognized that together they could provide support through sharing essential information and helping to find resources they need for their loved ones. This peer model continues to be at the heart of our work and has led RIPIN’s network to expand statewide. Today we have over 100 employees, providing peer to peer support across the lifespan within the areas of health, education and advocacy. We envision systems that produce better outcomes for the individual, family, child and community across Rhode Island.
Rhode Island Geriatric Education Center
Based out of the URI program in Gerontology, RIGEC is a consortium of partners dedicated solely to providing interprofessional geriatrics education and training. RIGEC is fully supported by the US Department of Health and Human Services, Health Resources and Services Administration through Grant #U1QHP28737-Geriatrics Workforce Enhancement Program. RIGEC aims to further improve health outcomes for older adults by integrating geriatrics into primary care delivery systems and engaging patients, families, and caregivers.
